Milliman IntelliScript will not have a copy of your report if you have not applied for insurance in the past seven years or authorized the release of information in connection with a claim.

You should get a copy and review it for accuracy, incorrect information in your report can result in your being denied life insurance, supplemental health insurance, or disability insurance and can result in higher premiums.

ExamOne Script Quest Diagnostics, Inc. is a consumer reporting agency for prescription history and pharmacy records related to an individual’s medical information. The report is used by insurance companies to determine your risk when you apply for coverage.
You are entitled to an annual copy of your report, contact them at their Consumer Report Request phone number 1-844-225-8047.
Request your copy and review it for errors. If errors are found, the Fair Credit Reporting Act (FCRA) requires ExamOne to investigate and correct any errors.
MIB Group
MIB Group is made up of insurance agencies. They gather and share medical and insurance related information.
The information they collect is used by insurance companies to determine the risk of insuring applicants. The Fair Credit Reporting Act requires MIB to provide consumers access to their reports annually.
They may have a report on you if you have applied for individual life or health insurance within the past seven years. To request a copy of your report, call 866-692-6901.
If you are applying for insurance request a copy of your reports with all three agencies before you apply, this allows you to review your report for errors and get them corrected beforehand. Errors on your report can lead to denial of insurance and higher premiums.
